First Nations Nurse Navigator role at Logan Hospital: creating more connected healthcare journeys

Metro South Health is recruiting an experienced Aboriginal and/or Torres Strait Islander Registered Nurse for an identified Nurse Navigator position at Logan Hospital.

Supporting patients across the healthcare journey

Healthcare systems can involve multiple teams, services and points of contact. This position is focused on reducing fragmentation and helping patients access the care and support they need throughout their journey.

The First Nations Nurse Navigator will provide advanced clinical leadership, person-centred care coordination and advocacy across primary, secondary and tertiary healthcare settings.

They will build trusted relationships with patients, families and carers while working closely with multidisciplinary healthcare teams and community partners.

The role will also collaborate with the Aboriginal and Torres Strait Islander Health Directorate, Cultural Coordination Team, the Institute for Urban Indigenous Health, Mob Link, local Elders and community organisations.

Clinical expertise and cultural knowledge

Metro South Health is looking for an experienced Registered Nurse with advanced clinical capability, strong communication skills and the confidence to work autonomously.

The successful applicant will understand how to navigate complex healthcare systems and connect patients with appropriate services and support. They will also bring an understanding of the social determinants of health and Primary Health Care principles.

Aboriginal and Torres Strait Islander peoples are diverse, and every patient brings their own identity, relationships, experiences, priorities and cultural context. This role provides an opportunity to combine clinical expertise with cultural knowledge and advocacy so that care is better connected and responsive to each person.

A relevant postgraduate qualification, enrolment in postgraduate study or previous experience in Aboriginal and Torres Strait Islander health will be highly regarded.
