  • Full-Time: $2,950 to $3,280 per fortnight

  • Part-Time: $38.81 to $43.16 per hour

About the Role:

We are looking for an Aboriginal and/or Torres Strait Islander person to join our Metro South Health team as a Home Support Assessor. You’ll provide in-home, non-clinical assessments of older clients, helping connect them to aged care services that align with their goals, independence, and wellbeing.

Key Responsibilities:

  • Conduct face-to-face client assessments using the Integrated Assessment Tool (IAT)

  • Develop client-led support plans, focused on reablement and independence

  • Link clients with services, carers, and supports in the community

  • Collaborate with health teams, agencies, and care providers

  • Write clear, professional reports and support documentation

Eligibility:

This is an Identified role. Applicants must identify as Aboriginal and/or Torres Strait Islander. A Certificate III in Home Care or Aged Care (or equivalent experience) is required. You must hold a valid driver’s licence and meet vaccination requirements.
